1. Fully accomplished Personal Data Sheet (PDS) with Work Experience Sheet and recent passport-sized
or unfiltered digital picture (CS Form No. 212, Revised 2025); digitally signed or electronically signed;
and be guided on “Guide to Filling Up the Personal Data Sheet” which can be downloaded at
www.csc.gov.ph;
2. Performance rating in the last rating period (if applicable);
3. Any proof of eligibility, report of rating/license/certificate (original copy, authenticated copy, certified
true copy, photocopy, scanned copy, or site/screen capture of the eligibility using Civil Service Eligibility
Verification System, Professional Regulation Commission’s (PRC)’s Licensure Examination and
Registration Information System (LERIS), or Supreme Court of the Philippines (SC) Lawyer’s List); for
Aircraft Mechanic applicants, CAAP License and Knowledge Test Report; for original appointment,
promotion, transfer, reappointment or reemployment;
4. Authenticated copy of Transcript of Records and Diploma;
5. PSA Birth Certificate and Marriage Contract (if applicable);
6. Complete Local Clearances (Barangay Clearance, Mayor’s Clearance, Police Clearance, RTC
Clearance, NBI Clearance, and D.I. PNP Clearance)
7. Register online through the PAF Civilian Human Resource Online Registration Form at
https://tinyurl.com/CivHR1Q2026.
